This is an event recording of the November 20, 2019 webinar with presenters Jenny Stranges (Interim Executive Director, Quest Community Health Centre) and Despina Tzemis (Program Manager, Quest Community Health Centre). This event was hosted by Niagara Connects in partnership with the Urgent Service Access Team (USAT) Project Management Team. Twenty-seven (27) participants joined this event from fourteen (14) different organizations across Niagara.

During this webinar, presenters shared how four organizations collaborate to support their inter-agency clients by developing coordinated care plans and care transition pathways. This has resulted in a 53% reduction in Emergency Department visits.
