Submitted to the Minister of Health and Long Term Care, this report outlines the current challenges in Ontario’s health system and puts forth recommendations and system enablers to support a system transformation. The findings in this report indicate that existing system capacity and program models are not optimally configured to address current or future ALC population needs, particularly for complex elderly patients who make up much of the ALC population.
