Bridges Community Health Centre (CHC) offers primary care, health promotion and community development to the communities of Fort Erie, Port Colborne and Wainfleet. Our inter-disciplinary team of health care providers use a social determinants of health approach to understand and respond to the unique needs of our clients, as well as our communities. In addition to programs and services focused on supporting and enabling the health of individuals, Bridges CHC is actively engaged in advocating for poverty reduction at the local, provincial and federal level. With healthy equity as an underlying principle, our advocacy efforts focus primarily on housing, a basic income, dental benefits for low income adults, food security and sustainable transportation. The work of Bridges CHC is also highlighted in the Sustainable Niagara Action Database.
