The Lived Experience Subcommittee of the Mental Health and Addictions Working Group, Niagara Ontario Health Team (Niagara OHT)/Équipe Santé Ontario Niagara (ESO-Niagara) is conducting a consultation about the important role that family and other natural supports play in helping people living with mental illness or addiction. Niagara-OHT (ESO-Niagara) members provide publicly-funded core mental health and addiction services.

If you are an adult providing support to someone 15 years of age or older with mental health and addiction concerns in Niagara, please complete this survey.  Responses will help mental health and addictions service providers in Niagara learn about how they can better support people helping someone close to them.
